Consent
I hereby declare that I am voluntarily offering my services for the Barsana Eye & Dental Camp to be held at Barsana, District Mathura, Uttar Pradesh, organized by the Bhaktivedanta Hospital & Research Institute and conducted by Sri Chaitanya Health & Care Trust, Thane.
I undertake to attend all necessary orientation programs conducted to understand the special circumstances of my allotted service, especially the possible biohazards while working in a medical service area. I take full responsibility for following all protocols set forth for my safety. I will attend the camp at my own responsibility. In the event of any untoward physical, mental, emotional or biohazardous trauma during my service and stay at the Barsana Eye & Dental Camp 2027, I shall not hold Bhaktivedanta Hospital & Research Institute or any of the organizers responsible.
Dress Code
- Men: Formal pants and shirt, or dhoti/pajamas and kurta. Jeans, T-shirts and casual clothing are strictly not allowed in the camp.
- Ladies: Punjabi dress or saree, as convenient for their service. Western clothing such as jeans, pants, skirts, sleeveless tops, frocks and transparent dresses is not allowed in the camp.
- Ladies are requested to keep their hair tied back or braided and not leave it untied.
Disclaimer
- Your registration will be confirmed by the Bhaktivedanta Hospital team after 15 October.
- The application fee of ₹1,000 is non-refundable.
- Kindly book your own tickets to and from Mathura.
- Bus transport will be available only in the morning at Mathura station for Rajdhani Express, August Kranti Express, Paschim Express and Garib Rath trains, between 7:00 AM and 10:00 AM on 1 February and 8 February.
- Volunteers must strictly adhere to the eight-hour seva schedule. If you find it difficult, you may join as a guest.
- Guests must pay ₹1,000 per day for shared accommodation.
- If a volunteer or guest wants accommodation other than that provided by the hospital, the actual accommodation cost must be borne by the volunteer or guest.
